rca-imageThe purpose of this Root Cause Analysis (RCA) Toolkit is to provide tools and information to guide a RCA. This RCA Toolkit outlines a standardized and systematic approach to identify opportunities for improvements that make patient care safer. The RCA process is intended to recognize system vulnerabilities so they can be eliminated, corrected, or mitigated. The publication, 2014 Prevention and Treatment of Pressure Ulcers: Clinical Practice Guideline by the European Pressure Ulcer Advisory Panel (EPUAP), National Pressure Ulcer Advisory Panel (NPUAP) and Pan Pacific Pressure Injury Alliance (PPPIA) was used as a framework in the development of this toolkit.

rca-formThe use of a RCA process will help a hospital/facility gain insight into the development of a pressure injury. This comprehensive evaluation includes an analysis of the influencing factors, a review of events and documentation prior to the pressure injury discovery. This process is not intended for the analysis of all hospital/facility-acquired pressure injuries (HAPI/FAPI) but rather for review of the development of a Stage 3, Stage 4, unstageable or deep tissue pressure injury (DTPI). This RCA Tool Kit may provide clarification of surrounding events, deviations from hospital/facility’s pressure injury prevention and treatment protocol. The result of this analysis may identify opportunities for improvement in the process of the hospital/ facility’s skin management program.
